Schedule: Full-time and Part-time positions available
Position Description:
The Advanced Practice Nurse Practitioner delivers care within our Palliative Care Program. The APNP will be responsible for providing care and services for hospice and palliative care patients. Visit patients wherever they reside which may include private residences, assisted living facilities, hospitals, long term care as well as group home environments. The APNP is available for face-to-face visits working independently yet in tandem with our Hospice Team.

Requirements:
Utah Nurse Practitioner license in good standing
Prescriptive authority
Reliable Transportation
Previous hospice experience is preferred but not required
