Rocky Mountain Care – Tooele is seeking a full-time SSW/CSW/LCSW to join our amazing team at Willow Springs in Tooele, UT.

You must possess a Bachelor’s degree, a Social Service Worker license in good standing with the State of Utah, or in the process of testing, be able to write and initiate care plans, have an outstanding bed-side manner, be able to set boundaries while maintaining compassion and understanding, and have the ability to see the “big picture”.
Must be organized, able to initiate and follow through with daily plans and tasks, be goal-oriented, meet deadlines, have a basic knowledge of Word, Microsoft, Excel, etc., and learn/use an EMR. In addition, you must be able to effectively communicate and maintain professional relationships with your team, residents/families, medical personnel, and outside health care entities.
Experience in a SNF is preferred but not necessary. Knowledge of PASRR and community resources is a plus. We are happy to train the right person!
The primary purpose of this position is to plan, organize, develop, and direct the overall operations of the Social Services Department and its employees. Among some of the responsibilities, you will assist with abuse investigations, DC planning, family/resident conferences, MDS assessments, help mediate difficult situations, and follow RMC Policy & Procedures and SNF Rules & Regulations.
