The Home Health Visit Nurse is responsible and accountable for making decisions that are based on the individuals educational preparation and nursing experience and for providing nursing services as ordered on a patient plan of care.


  • Records all pertinent observations and treatment as per Agency Policy
  • Provide for the emotional comfort, physical comfort and safety of the client(s) assigned
  • Participates in case conferences, meetings and performance activities as assigned
  • Complies with all State and Regulatory acts
  • Conducts self in a professional manner at all times in all situations
  • Responsible for visiting patients within the assigned time frame
  • Report any unusual or abnormal findings to RN Case Manager
  • Administer medications and document therapeutic effects
  • Take vital signs (temperature, blood pressure, pulse and respiration)
  • Prepare and administer injections and/or enemas
  • Provide care per plans of care and MD order
  • Perform duties as a Licensed Practical Nurse or Registered Nurse in accordance with Scope of Practice and Utah Law


  • Must be a graduate of any approved school of nursing and currently licensed in the State of Utah.
  • Must have excellent communication skills
  • Must make patient care a top priority
  • Must have a positive attitude and demeanor
  • Must have reliable transportation
