We are now hiring a FT SSW/CSW/LCSW to join our team at Willow Springs in Tooele, UT as the Director of Social Services.

Willow Springs is a beautiful 112 bed Skilled Nursing Facility with a fast-paced environment that relies on teamwork and a good attitude to get the best possible outcomes for both our Long-Term and Short-Term residents.

Minimum requirements:

You must possess a Bachelor’s degree, a Social Service Worker license in good standing with the State of Utah, or in the process of testing, be able to write and initiate care plans, have an outstanding bed-side manner, be able to set boundaries while maintaining compassion and understanding, and have the ability to see the “big picture”.

Minimum skills:

You must be organized, able to initiate and follow through with daily plans and tasks, be goal oriented, meet deadlines, have a basic knowledge of Word, Microsoft, Excel, etc., and learn/use an EMR. In addition, you must be able to effectively communicate and maintain professional relationships with your team, residents/families, medical personnel, and outside health care entities.


Experience in a SNF is preferred but necessary. Knowledge of PASRR and community resources is a plus. We are happy to train the right person!


The primary purpose of this position is to plan, organize, develop, and direct the overall operations of the Social Services Department and its employees. Among some of the responsibilities, you will assist with abuse investigations, DC planning, family/resident conferences, MDS assessments, help mediate difficult situations, and follow RMC Policy & Procedures and SNF Rules & Regulations.
